Thank you.
FACULTY: PLEASE READ IN ALL CLASSES
In 2003, New York State passed a Public Health Law requiring colleges and universities
to distribute information about meningococcal disease and vaccination to all
college and university students. All students must complete a Health History
and Meningitis Acknowledgement Form. According to the law, no institution can
permit students who have not complied with the law to attend in excess of 30
days. Students who have not complied with the requirement to complete the Information
Response Form will be blocked from registration for future semesters and may
be suspended.
Students may obtain a form from the SCCC website (http:www.sunysuffolk.edu)
by going to “Student Services”, then “Health Services”
and then “Meningitis Requirements.”
The Health History and Meningitis Acknowledgement Form may be printed, completed
and either faxed to 451-4874 or brought into the Health Services Office in Kreiling
Hall, Rm. M106. Call 631-451-4047 for more information.
Thank you for your attention in this important matter.
